To prepare for your consultation, we recommend the following: -Complete any patient forms ahead of time (available on our website). -Bring a valid ID, insurance card, and a list of current medications. -If you have previous imaging results (X-rays, MRIs, etc.), please bring those along. -Write down any specific questions or concerns you want to discuss with your provider.
4How do I schedule an appointment?
We are a referral-based practice, so patients will need to have a referral from their primary care physician or specialist. Once we receive and review your records, our Referral Department will reach out to schedule your appointment. Alternatively, you can call our office and press "New Patient" for further assistance with the referral process.
5Will I need any special tests or imaging?
Depending on your condition, we may recommend diagnostic tests such as X-rays, MRIs, or CT scans to better understand the cause of your pain. Our team will discuss the need for any tests during your consultation, and we’ll coordinate the scheduling for any necessary procedures.
